This presentation will include a brief description of the history and structure of a CKD Navigator program in Vermont. Clinical outcomes achieved by the program will be reviewed. Estimates of the financial impact of these outcomes will be presented.

Dr. Jeffrey M. Rimmer received his MD from the University of Connecticut Health Center, completed a medical residency at the Rhode Island Hospital, Brown University and and a Nephrology Fellowship at The Robert Larner, M.D. College of Medicine at The University of Vermont. He developed the University of Vermont Medical Center dialysis system over the past forty years where he remains the Senior Medical Director. During this time he was also active in research in areas of dialysis, ckd and hypertension.He aslo wass an educator working with medical students, residents and fellows. With a long interest in improving care in CKD, he received a grant from OneCare Vermont to develop a CKD Navigator program with the goal of addressing the needs of persons with advanced kidney disease. That program continues within the the University of Vermont Health Network.
