Healthy Communities: Supporting Patient Empowerment and Access to Care in Kidney Disease

The Centers for Medicare & Medicaid Services (CMS) key goals for 2025 include efforts at increasing patient empowerment through improved patient access to information and choice in how they receive care. The National Forum of ESRD Networks has recently published a toolkit that focuses on building healthier communities for those patients living with kidney disease and will present on ways to overcome the myriad health disparities that they face. Topics will include ways to identify and address specific health disparities in this population as well as a review of interventions that have improved the care of specific populations of patients living with kidney disease. Lastly, we will review our experience with efforts to improve patient-provider collaboration in this complex environment.

Daniel Landry, DO

President

National Forum of ESRD Networks

Dr. Daniel Landry is a private practice nephrologist working for Kidney Care & Transplant Services of New England, a group composed of 14 nephrologists, transplant/vascular access surgeons and advanced practitioners located in Springfield, MA. They have been involved in the ESCO since its inception in 2017 and now are involved in the Kidney Care First value-based payment model aimed at improving the outcomes of people living with kidney disease. He currently serves as the president of the National Forum of ESRD Networks as well as the chair of the medical review board for ESRD Network 1. He is the division chief and fellowship program director for nephrology at Baystate Medical Center and the medical director for their acute dialysis and critical care nephrology programs. In the outpatient world, he is the medical director of Fresenius outpatient and home dialysis programs in East Springfield, MA. Dr. Landry’s clinical and research interests include population health and value-based care delivery models, as well as efforts to improve the education of our next generation of nephrology physicians and advanced practice providers. 

Donald A. Molony, MD

Professor of Medicine

Donald A. Molony, M.D. is a Regents Distinguished Teaching Professor of the University of Texas System and Professor of Medicine in the Division of Renal Diseases and Hypertension and in the Center for Clinical Research and Evidence-Based Medicine at the University of Texas McGovern Medical School in Houston. He has served as Course-Director for Problem Based Learning and Fundamentals of Clinical Medicine, core medical school courses at UT Houston. He has served as Chair of the Medical School Faculty Senate and Chair of University of Texas System Faculty Advisory Council. He is a member of the UT-Houston Medical School Academy of Master Educators, and The University Of Texas Shine Academy of Health Sciences Education.  He has served on the curriculum committee and chairs the UTHealth Chemical Safety Committee.  He has served as director of Evidence-based Medicine for MHH-TMC and on the Memorial Hermann Hospital System Health Informatics Committee. He is Past-Chair of the ESRD Network 14 Medical Review Board, Past-Chair of the Medical Advisory Council of the Forum of ESRD Networks, and Past-President of the Forum.  He currently serves as Network 14 representative on the Forum Medical Advisory Council.   He has represented the Forum on the Kidney Care Quality Alliance. He has served on the Renal Physicians Association Educational Committee since 2017. Dr. Molony is co-edited with Dr. Jonathan Craig of Adelaide, Australia and Giovanni Strippoli of Bari, Italy of the textbook Evidence-based Nephrology 2nd Edition (Published 2023). He has served as CME Section Editor for the American Journal of Kidney Disease, as an associate editor of Advances in Chronic Kidney Diseases, as section editor of Clinical Nephrology, and as editor of Literature Watch for Dialysis and Transplantation. Dr. Molony’s current research focuses on the influence of environmental toxicants and other factors on the progression of chronic kidney disease and on the application of the principles of evidence-based medicine and epidemiology to the delivery of health care, in particular, how new advances in science and clinical trials can be seamlessly and optimally incorporated into the evidence-based Nephrology practice. He serves as co-PI on a NIH sponsored research project examining the effects of metals including uranium on CKD and cardiovascular disease in a Native-American population. Dr. Molony received his BS in biology from Yale and his MD from the University of California, San Diego. He completed his internship and residency in 1981 in the Department of Internal Medicine at the University of Texas Southwestern Medical School and Parkland Memorial Hospital in Dallas, Texas. His research and clinical fellowships were completed at the University of Texas Southwestern Medical School (1981-1984). He has been a faculty member at the University of Texas McGovern Medical School-Houston since 1984.

Mary Baliker, BS

The National Forum of ESRD Networks

Mary Baliker has a degree from the University of Wisconsin in Public Health and Psychology. Her professional work experience over the past 30 years includes innumerable international, national, and state patient centered initiatives. She has worked with many healthcare organizations; including the University of Wisconsin School of Medicine & Public Health, and Long Beach Memorial Hospital, she has served as an organ procurement coordinator, clinical research transplant coordinator, outreach transplant education coordinator, and healthcare administration. Currently she works as a healthcare consultant as well as an inspirational speaker, educating, mentoring, and informing attendees with her stories and personal journey. In addition to Mary’s consulting and volunteer work she has written a book which can be found on Amazon, “Maria Never Gives Up” “Are You Like Me?” she hopes to help other children and their families in their struggle with a health crisis. Mary was diagnosed with kidney disease at the age of 9 and has experienced in-center hemodialysis various times in her life. At 17, she had a kidney transplant from her brother, Doug. Since then, she has had three more transplants, the last of which was in 1999. As a result of this life-long experience, she cherishes her life and possesses a strong interest to help improve healthcare, patient outcomes and quality of life. She is passionate and committed to helping kidney patients. Mary has extensive experience in collaborating with Pharmaceutical Companies, Nonprofits, and Universities. She also participates in technical expert panels led by the Center for Medicare & Medicaid Services and been a peer reviewer for the US Department of Defense medical research programs, and Patient-Centered Outcomes Research Institute. She has published peer reviewed publications and written articles sharing the patient experience. Mary’s diverse background has inspired her to become a fierce healthcare advocate. She serves on: 1.
