For many pediatric patients, weekly CVL dressing changes can be a source of significant anxiety. This session explores how a multidisciplinary team partnered with patients and families to redesign this experience through individualized care planning.
By incorporating patient-specific preferences—such as stress management strategies, timing, positioning, caregiver support, and comfort measures—teams created tailored plans to support each child during dressing changes. These plans are continuously adapted to ensure ongoing success and responsiveness to patient needs.
This approach has resulted in a notable improvement in patient experience, engagement, and procedural success. Participants will leave with actionable strategies to implement personalized, patient-centered interventions within their own practice settings.

In this role, she partners closely with interdisciplinary teams to advance quality, safety, and patient-centered care across the continuum of kidney services.
Cassie has worked in nephrology since 2010, beginning her career in adult dialysis before transitioning to pediatric care in 2019. Her clinical and leadership experience spans both inpatient and outpatient settings, with a focus on program development, care standardization, and improving outcomes for patients with complex kidney conditions.
Her passion for kidney care is deeply personal, shaped by her father’s experience with kidney failure and dialysis, as well as that of other family members. This webinar will provide an overview of an effective model of concurrent hospice and dialysis that includes (1) timely goals-of-care conversations; (2) an interdisciplinary approach; (3) clear guidelines to ensure smooth transitions for patients and families; and (4) discussion of the need for Hospice payment policy that supports concurrent care, such as the Concurrent Care for Comfort Act, a bipartisan bill that will address this issue and ensure that ESKD patients have the same access to hospice as Medicare beneficiaries with other end stage diagnoses.
Hospice care is meant to provide comfort and dignity for people nearing the end of life, but for patients with kidney failure this decision is much more complicated. Currently, patients on dialysis must make the decision to stop dialysis if they want to receive hospice services. In Western Pennsylvania, we have piloted a concurrent hospice and dialysis program for eight years offering more individuals the opportunity to pass away in comfort with their family compared to most Medicare beneficiaries on dialysis. She is dual-trained in palliative medicine and nephrology. Her academic passion is improving palliative care within nephrology through education and innovative practice models. She developed a nationally recognized communication training, NephroTalk, designed for teach nephrology fellows and nephrologists palliative care communication skills for treatment decision-making and end of life care in patients with kidney disease. Her current research interests are focused on innovative programs to improve palliative care and hospice services in patients with kidney disease at end of life.

This presentation discusses the basics of policy development and implementation. Topics covered include: The importance of having policies as an organization, how to identify an issue that requires a policy, how to form an inter-disciplinary policy committee, where to find the necessary resources to write an effective policy, how to develop and write a new policy, and most importantly how to effectively implement a policy within the organization. Attendees will begin writing a policy during this presentation so come with ideas!
