    One of the biggest things we can do to make our DOH surveys easier is to develop policies that are evidenced based, clear, and support our staff processes. This presentation discusses the basics of policy development and implementation.  Topics covered include: The importance of having policies as an organization, how to identify an issue that requires a policy, how to form an inter-disciplinary policy committee, where to find the necessary resources to write an effective policy, how to develop and write a new policy, and most importantly how to effectively implement a policy within the organization.  Attendees will begin writing a policy during this presentation so come with ideas!

Now, with almost 24 years of dialysis experience, her career has included in-center hemodialysis, 10 + years working in peritoneal and home hemodialysis, 15+ years of  policy and procedure development and review as chair and co-chair for both in-center and home dialysis, and various other task forces to improve outcomes for patients.

    Danilo Concepcion has been a dialysis practitioner since 1975 and is currently the Operations Manager, Renal Services for Providence St. Joseph Hospital, Orange, CA. Prior to his entering the dialysis field, he was a corpsman in the Navy during the Vietnam era. He is experienced in the clinical, technological, research and administrative areas of dialysis.     Dr. Jeffrey M. Rimmer received his MD from the University of Connecticut Health Center, completed a medical residency at the Rhode Island Hospital, Brown University and and a Nephrology Fellowship at The Robert Larner, M.D. College of Medicine at The University of Vermont. He developed the University of Vermont Medical Center dialysis system over the past forty years where he remains the Senior Medical Director. During this time he was also active in research in areas of dialysis, ckd and hypertension.He aslo wass an educator working with medical students, residents and fellows. With a long interest in improving care in CKD, he received a grant from OneCare Vermont to develop a CKD Navigator program with the goal of addressing the needs of persons with advanced kidney disease. That program continues within the the University of Vermont Health Network.

    Daniel Landry, DO

    Dr. Daniel Landry is a private practice nephrologist working for Kidney Care & Transplant Services of New England, a group composed of 14 nephrologists, transplant/vascular access surgeons and advanced practitioners located in Springfield, MA. They have been involved in the ESCO since its inception in 2017 and now are involved in the Kidney Care First value-based payment model aimed at improving the outcomes of people living with kidney disease. He currently serves as the president of the National Forum of ESRD Networks as well as the chair of the medical review board for ESRD Network 1. He is the division chief and fellowship program director for nephrology at Baystate Medical Center and the medical director for their acute dialysis and critical care nephrology programs. In the outpatient world, he is the medical director of Fresenius outpatient and home dialysis programs in East Springfield, MA. Dr. Landry’s clinical and research interests include population health and value-based care delivery models, as well as efforts to improve the education of our next generation of nephrology physicians and advanced practice providers. 

    Donald A. Molony, MD

    Donald A. Molony, M.D. is a Regents Distinguished Teaching Professor of the University of Texas System and Professor of Medicine in the Division of Renal Diseases and Hypertension and in the Center for Clinical Research and Evidence-Based Medicine at the University of Texas McGovern Medical School in Houston. He has served as Course-Director for Problem Based Learning and Fundamentals of Clinical Medicine, core medical school courses at UT Houston. He has served as Chair of the Medical School Faculty Senate and Chair of University of Texas System Faculty Advisory Council. He is a member of the UT-Houston Medical School Academy of Master Educators, and The University Of Texas Shine Academy of Health Sciences Education.  He has served on the curriculum committee and chairs the UTHealth Chemical Safety Committee.  He has served as director of Evidence-based Medicine for MHH-TMC and on the Memorial Hermann Hospital System Health Informatics Committee. He is Past-Chair of the ESRD Network 14 Medical Review Board, Past-Chair of the Medical Advisory Council of the Forum of ESRD Networks, and Past-President of the Forum.  He currently serves as Network 14 representative on the Forum Medical Advisory Council.   He has represented the Forum on the Kidney Care Quality Alliance. He has served on the Renal Physicians Association Educational Committee since 2017. Dr. Molony is co-edited with Dr. Jonathan Craig of Adelaide, Australia and Giovanni Strippoli of Bari, Italy of the textbook Evidence-based Nephrology 2nd Edition (Published 2023). He has served as CME Section Editor for the American Journal of Kidney Disease, as an associate editor of Advances in Chronic Kidney Diseases, as section editor of Clinical Nephrology, and as editor of Literature Watch for Dialysis and Transplantation. Dr. Molony’s current research focuses on the influence of environmental toxicants and other factors on the progression of chronic kidney disease and on the application of the principles of evidence-based medicine and epidemiology to the delivery of health care, in particular, how new advances in science and clinical trials can be seamlessly and optimally incorporated into the evidence-based Nephrology practice. He serves as co-PI on a NIH sponsored research project examining the effects of metals including uranium on CKD and cardiovascular disease in a Native-American population. Dr. Molony received his BS in biology from Yale and his MD from the University of California, San Diego. He completed his internship and residency in 1981 in the Department of Internal Medicine at the University of Texas Southwestern Medical School and Parkland Memorial Hospital in Dallas, Texas. His research and clinical fellowships were completed at the University of Texas Southwestern Medical School (1981-1984). He has been a faculty member at the University of Texas McGovern Medical School-Houston since 1984.

    Mary Baliker, BS

    Mary Baliker has a degree from the University of Wisconsin in Public Health and Psychology. Her professional work experience over the past 30 years includes innumerable international, national, and state patient centered initiatives. She has worked with many healthcare organizations; including the University of Wisconsin School of Medicine & Public Health, and Long Beach Memorial Hospital, she has served as an organ procurement coordinator, clinical research transplant coordinator, outreach transplant education coordinator, and healthcare administration. Currently she works as a healthcare consultant as well as an inspirational speaker, educating, mentoring, and informing attendees with her stories and personal journey. In addition to Mary’s consulting and volunteer work she has written a book which can be found on Amazon, “Maria Never Gives Up” “Are You Like Me?” she hopes to help other children and their families in their struggle with a health crisis. Mary was diagnosed with kidney disease at the age of 9 and has experienced in-center hemodialysis various times in her life. At 17, she had a kidney transplant from her brother, Doug. Since then, she has had three more transplants, the last of which was in 1999. As a result of this life-long experience, she cherishes her life and possesses a strong interest to help improve healthcare, patient outcomes and quality of life. She is passionate and committed to helping kidney patients. Mary has extensive experience in collaborating with Pharmaceutical Companies, Nonprofits, and Universities. She also participates in technical expert panels led by the Center for Medicare & Medicaid Services and been a peer reviewer for the US Department of Defense medical research programs, and Patient-Centered Outcomes Research Institute.
